Please note: Full payment or estimated insurance co-payment is due at the time of service

Treatment decisions are made in the best interest of your oral health based on your individual needs and risk factors. Insurance coverage often falls under a set of parameters that may or may not take into consideration your particular condition or situation.

You are responsible for the fees charged by our office, no matter what your insurance coverage may be. It is very important you understand your insurance coverage.  Our dedicated team will make every effort to help answer questions you may have about insurance.

As a courtesy to you, we will bill your insurance company and track claims.

Please keep us informed of any changes to your insurance plan.  Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility.
